Background to the Study
Globally, healthcare systems are struggling with increasing demands on outpatient services, which place pressure on hospitals and healthcare providers to deliver efficient and patient-centered care. In many countries, long waiting times remain a persistent problem, often resulting in overcrowded clinics, patient dissatisfaction, and compromised service delivery. For instance, in China, some outpatient clinics have recorded registration and waiting times of over 13 hours before the adoption of digital scheduling systems (Cao et al., 2011). This issue is not confined to low-income countries; even in the United Kingdom, inefficiencies in patient scheduling have led to millions of pounds lost due to surgical cancellations, prompting policy recommendations to better utilize digital appointment technologies (The Times, 2025). Nigeria, like many other developing nations, experiences a critical imbalance in its healthcare delivery system. One major concern is the country’s low physician-to-patient ratio. According to the World Bank (2022), Nigeria had only 3.94 Physicians per 1,000 people as of 2022 far below the World Health Organization’s recommended benchmark of 10 doctors per 1,000 population. This shortfall, in combination with the mass emigration of medical professionals due to poor remuneration, insecurity, and inadequate infrastructure (Onah et al., 2022), places immense strain on healthcare personnel and limits the system’s capacity to handle rising patient volumes effectively. This imbalance has led to a surge in waiting times, particularly in Outpatient Departments (OPDs), where patients are often forced to wait for extended periods before receiving care. In tertiary hospitals within Akwa Ibom State, such as the University of Uyo Teaching Hospital, the scenario is not much different. A hybrid appointment booking system is currently in use, involving both digital databases and traditional paper records. While the hospital has partnered with Health-in-a-Box Solution Nigeria Limited to manage digital services such as registration and billing, clinical consultations still rely on paper-based documentation. This results in systemic inefficiencies, including overbooking, long queues, and patient dissatisfaction. The current system also suffers from inadequacies in appointment reminders. Health Information Management staff typically send SMS reminders to patients using mobile phones. This manual method is not only inefficient but also susceptible to human error and irregularities. Consequently, high rates of missed appointments, or no-shows, are frequently recorded. A study by Omokanye et al. (2021) found that missed appointments in Ilorin, Nigeria, reached as high as 38%, with long waiting times and the absence of effective reminder systems cited as key contributing factors. These missed appointments further exacerbate the problem of clinic overcrowding, as rescheduled patients add to the already overburdened system. A digital appointment system has been proposed as a viable solution to these challenges. In many developed countries, the transition from manual to digital systems has led to measurable improvements in service delivery. Cao et al. (2011) demonstrated that a web-based appointment system significantly reduced waiting times and enhanced patient satisfaction in a Chinese tertiary hospital. Similarly, a systematic review by Zhao et al. (2017) concluded that digital systems consistently improved operational efficiency and reduced no-show rates across multiple healthcare settings. These systems provide capabilities such as real-time booking, automatic reminders via SMS or email, and the ability for healthcare providers to manage schedules dynamically. By streamlining the appointment process, digital systems not only ease administrative burdens but also improve patient flow and reduce the risk of disease transmission in overcrowded waiting areas. Despite these proven benefits, implementing digital appointment systems in Nigeria poses significant challenges. Infrastructure deficits, including erratic electricity supply and limited internet access, remain critical barriers. Furthermore, many healthcare workers lack adequate training in digital technologies, which could impede system adoption and integration into existing workflows. In the University of Uyo Teaching Hospital, for instance, the existing digital infrastructure is underutilized due to poor coordination between departments and a lack of standardized operating procedures. There is also the risk of initial disruptions to workflow during the transition phase, as staff and patients alike adjust to the new system. Training and change management are therefore essential components of any successful implementation strategy. Moreover, patient awareness and access to digital tools also play a crucial role in determining the feasibility of such systems. While smartphone penetration in Nigeria is increasing, a significant proportion of the population particularly in rural areas may not have reliable access to the internet or the digital literacy required to navigate online booking platforms. As observed in a study by Zhang et al. (2014), awareness campaigns and educational interventions are often necessary to increase the adoption rate of digital appointment systems, especially among older and less tech-savvy patients. The need for an efficient appointment system is further underscored by evidence linking long waiting times to poor health outcomes. A study by Nkosi et al. (2019) found that overcrowded hospital environments contributed to the spread of infectious diseases, such as respiratory illnesses, through mechanisms such as coughing and sneezing. These risks are magnified in OPDs where physical distancing is difficult to enforce. In light of this, the implementation of a digital appointment system could play a vital role in infection control by reducing crowd density and improving patient scheduling. The evolving expectations of patients also make the case for digital innovation in healthcare delivery. As digital technologies become more pervasive across various sectors, patients are beginning to expect the same level of convenience and efficiency from healthcare providers. A report by Healthcare Success (Gandolf, n.d.) emphasized that modern patients are no longer willing to endure long wait times and inefficient scheduling. Instead, they expect prompt service, timely communication, and digital tools that enhance their overall healthcare experience. A digitally enabled appointment system aligns with these expectations and has the potential to significantly boost patient satisfaction and institutional credibility. In sum, while the use of digital appointment systems is not without its challenges, there is compelling evidence to support their feasibility and potential effectiveness in tertiary hospitals in Akwa Ibom State. The burden of manual scheduling systems is evident in the overcrowded clinics, excessive wait times, and rising rates of patient dissatisfaction. By leveraging technology, these institutions can streamline operations, optimize resource utilization, and ultimately improve healthcare outcomes for the populations they serve. However, any transition must be guided by a comprehensive understanding of the local context, a commitment to staff training, and investment in necessary infrastructure. Only then can the full benefits of digital appointment systems be realized in the healthcare sector in Akwa Ibom State and beyond.
